Refund Policy
Medical 360 HC PLLC DBA StackMD
Effective Date: January 15, 2026
All sales of prescription medications through StackMD are final. No refunds are issued once a prescription medication has been dispensed and delivered. This policy is established in compliance with applicable federal and state laws governing the dispensing, handling, and return of prescription medications. By purchasing prescription medications through StackMD, you acknowledge and agree to the terms below.
Why Prescription Medications Cannot Be Returned
In accordance with FDA guidelines governing returned medications (21 CFR § 205.50), StackMD does not accept returns for prescription medications once they have left the dispensing pharmacy’s control. This applies to all prescription medications dispensed through our services, including compounded medications.
Once dispensed and delivered, medications cannot be returned due to the risk of contamination, improper storage, or tampering — regardless of whether the medication has been opened or used. This protects patient safety and meets federal and state regulatory requirements.
Please review all prescription details carefully before your order is fulfilled. Any concerns about order accuracy should be reported to us before dispensing whenever possible.
Compounded Medications
Compounded medications are prepared specifically for you based on your provider’s prescription. Because they are custom-formulated for individual patient use, they cannot be restocked, resold, or reused. All compounded medication orders are therefore non-refundable once the compounding pharmacy has begun preparation, regardless of whether the order has shipped.

Limited Exceptions
While we cannot accept returned medications, StackMD will work with patients to resolve qualifying issues in the following circumstances:
1. Dispensing Errors
If an error occurs due to StackMD or the dispensing pharmacy — including incorrect medication, wrong dosage, or labeling errors — a resolution may be considered.
You must notify us within 7 calendar days of receipt and provide:
- Photos of the medication, including packaging and labels
- Proof of purchase
- Any related prescription communication
Verified errors may be resolved through replacement of the correct medication, correction of documentation, or other actions permitted by law.
2. Damaged or Compromised Shipments
If your medication arrives visibly damaged or compromised in transit, report the issue within 72 hours of delivery and include:
- A description of the issue
- Clear photos of the packaging and contents
StackMD is not responsible for improper storage, handling, or misuse after delivery.
Review and Resolution Process
All claims are reviewed in coordination with the dispensing pharmacy and, where applicable, the shipping provider. Verified claims will be resolved through replacement or other remedies permitted by law. Late or incomplete submissions may be denied.
Legal Compliance
This policy is established in accordance with:
- FDA regulations, including 21 CFR § 205.50
- New York State Board of Pharmacy requirements
- DEA regulations, where applicable
Patient Responsibilities
Patients are responsible for:
- Verifying all prescription and order details before purchase
- Storing and handling medications according to the instructions provided
- Following all prescribing instructions from their provider
StackMD is not responsible for outcomes resulting from misuse, improper storage, or non-compliance with prescribing instructions after delivery.
