What Is Tirzepatide

A Higher-Potency Option for Medical Weight Loss

Tirzepatide is a once-weekly injectable medication that acts as both a GLP-1 receptor agonist and a GIP (glucose-dependent insulinotropic polypeptide) receptor agonist — making it the first dual-hormone weight loss therapy of its kind. This is what separates tirzepatide from single-pathway options like semaglutide weight loss treatment.

By targeting two separate metabolic pathways simultaneously, tirzepatide may deliver weight loss outcomes that exceed those of GLP-1-only medications. It is FDA-approved for chronic weight management under the brand name Zepbound, and for type 2 diabetes under Mounjaro — both manufactured by Eli Lilly.

In the landmark SURMOUNT-5 head-to-head trial published in the New England Journal of Medicine in May 2025, tirzepatide produced nearly 50% greater weight loss than semaglutide in adults with obesity over 72 weeks. StackMD offers tirzepatide treatment with physician oversight at our Long Island practice.

How Tirzepatide Works

1

Dual Receptor Activation

Activates both GLP-1 and GIP receptors — two independent pathways that regulate hunger, satiety, insulin secretion, and energy storage.

2

Superior Appetite Suppression

The combination of GLP-1 and GIP action is designed to reduce appetite more significantly than GLP-1 alone.

3

Enhanced Insulin Sensitivity

GIP agonism may strengthen insulin release and improve insulin sensitivity — important for metabolic dysfunction and prediabetes.

4

Slowed Gastric Emptying

Prolongs fullness between meals by slowing the movement of food through the digestive tract.

Clinical Evidence

Head-to-Head: Tirzepatide vs. Semaglutide

In the SURMOUNT-5 trial published in the New England Journal of Medicine (May 2025), adults with obesity but without type 2 diabetes were randomized to the maximum tolerated dose of tirzepatide or semaglutide for 72 weeks. The results consistently favored tirzepatide.

Tirzepatide produced an average weight loss of 20.2% versus 13.7% with semaglutide — a difference of nearly 50%. Tirzepatide was also superior across every secondary endpoint including waist circumference reduction and achieving 10%, 15%, 20%, and 25% weight loss thresholds. Individual results may vary.
